Please provide accurate and complete information to ensure safe and effective treatment.

Your physiotherapist will conduct a hands-on physical examination which may include:

- Palpation of muscles, joints, and soft tissues

Treatment Plan Discussion
Based on the assessment findings, your physiotherapist will discuss the diagnosis, proposed treatment plan, expected number of sessions, treatment goals, and home exercise programme with you before proceeding.
3. Treatment Procedures
Aspire Wellness offers a range of physiotherapy and wellness treatments. Depending on your condition, treatment may include one or more of the following:

- Therapeutic exercises and stretching programmes
- Electrotherapy modalities (ultrasound, TENS, electrical stimulation)
- Dry needling and trigger point therapy

- Heat or cold therapy
- Postural and ergonomic education
- Home exercise and self-management programmes
Your physiotherapist will explain each treatment technique before applying it and will seek your verbal consent before introducing any new treatment modality during your sessions.

Possible Risks

- In rare cases, aggravation of existing symptoms

5. Your Rights as a Patient
As a patient of Aspire Wellness, you have the right to:
- Receive respectful, dignified, and non-discriminatory care at all times
- Be informed about your condition, treatment options, and progress in clear, understandable language
- Ask questions and receive honest, thorough answers about any aspect of your care
- Refuse any treatment or procedure without penalty or prejudice to future care
- Request a different practitioner if desired
- Have a chaperone or companion present during assessment or treatment
- Have your personal information and medical records kept confidential
- Access your patient records upon request
- File a complaint through our formal complaints procedure
- Seek a second opinion from another healthcare provider
6. Patient Responsibilities
To ensure the best possible outcomes from your treatment, we ask that you:
- Provide accurate and complete information about your medical history, current medications, and health status
- Inform your physiotherapist of any changes in your condition or health between sessions
- Follow the prescribed treatment plan and perform recommended home exercises
- Communicate openly about any pain, discomfort, or concerns during treatment
- Attend scheduled appointments on time or provide adequate notice for cancellations
- Treat our staff and other patients with courtesy and respect
- Settle accounts as per our payment and billing policy
7. Minors & Legal Guardians
For patients under the age of 18, the following applies:
- Written consent must be obtained from a parent or legal guardian before any assessment or treatment
- A parent or guardian must be present during the initial assessment
- For subsequent sessions, a parent or guardian must remain on the premises unless prior written arrangement has been made
- Where appropriate, we will also seek the assent of the minor patient and explain treatment in age-appropriate language
8. Withdrawal of Consent
You may withdraw your consent for treatment at any time, without providing a reason. If you wish to discontinue treatment:
- Simply inform your physiotherapist verbally or in writing
- Treatment will cease immediately upon withdrawal of consent
- Your decision to withdraw consent will not affect your right to receive future care from Aspire Wellness
- Your physiotherapist may discuss the implications of stopping treatment, but the final decision always rests with you
